We need to sleep every night to give our body important R&amp;R, <a href=\"https:\/\/www.sleepfoundation.org\/articles\/why-do-we-need-sleep\">among many other things<\/a>. And research has shown how getting a <a href=\"https:\/\/www.sleep.org\/articles\/sleep-quantity-different-sleep-quality\/\">good night\u2019s sleep<\/a> is crucial for our <a href=\"https:\/\/blogs.imperial.ac.uk\/ighi\/2019\/05\/13\/lets-talk-about-young-peoples-mental-health\/\">mental health<\/a> and wellbeing.<\/p>\n<p>When we sleep well, we\u2019re more likely to have greater concentration, be in a better mood and get things done. In contrast, when we don\u2019t, we can really see and feel the opposite effect. While we all have a poor night\u2019s sleep from time to time, we know that people <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1389945717300060?via%3Dihub\">in prison<\/a> and forensic <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6189980\/\">mental health hospitals<\/a> in the UK struggle more than most. Yet sleep has seemingly stayed off the radar and isn\u2019t prioritised in these areas.<\/p>\n<p>That\u2019s why we believed it was important to get together and tell people in these settings about the importance of sleep. We also wanted to inform our brand-new research proposal on managing sleep problems better in secure environments.<\/p>\n<h2>Giving importance to sleep in a secure hospital<\/h2>\n<p>Our research group originally came together after the Forensic Aspects of Sleep: Research and Development conference in Middlesbrough. We applied for and won around \u00a310,000 from the NHS Research Capability Funding to establish a new cross-disciplinary forensic research group.<\/p>\n<p>The resulting team is a collaboration between Tees, Esk Wear Valley (TEWV) NHS Foundation Trust, University of York and Imperial College London. People in our group have varied professional and lived experiences, including psychiatry, clinical academia, sleep, senior psychiatric nursing and service users. We\u2019ve met five times in the last year in a medium secure hospital in Middlesbrough, where we recently held an event to engage the public about sleep.<\/p>\n<figure id=\"attachment_2142\" aria-describedby=\"caption-attachment-2142\" style=\"width: 640px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"768\" class=\"size-large wp-image-2142\" src=\"https:\/\/blogs.imperial.ac.uk\/ighi\/files\/2020\/02\/Lindsay-sleep-1024x768.jpg\" alt=\"The sleep research group\" \/><figcaption id=\"caption-attachment-2142\" class=\"wp-caption-text\">Our research group (Lindsay third from left)<\/figcaption><\/figure>\n<h2>Engaging inpatients in sleep<\/h2>\n<p>On 21st January 2020, we ran a half-day forensic sleep public engagement event within the secure grounds of the forensic psychiatric hospital. We wanted to increase awareness of the importance of sleep to TEWV staff, but also to include as many voices as possible in taking our work forward to inform our research.<\/p>\n<p>The event brought together patients, clinicians, senior managers and commissioners. We set up four different areas that people were free to explore. These were: 1) our research group\u2019s journey so far, 2) how we measure sleep, 3) why sleep is important and 4) Imperial\u2019s \u2018<a href=\"https:\/\/www.voice-global.org\/news-blogs\/2018\/may\/the-ppi-caf%C3%A9-open-for-ideas\/\">People\u2019s Research Caf\u00e9<\/a>\u2019 to inform our next steps.<\/p>\n<p>The Caf\u00e9 gives people the chance to meet researchers and exchange ideas on projects, and it was the first time it had been run in a secure environment. Both patients and staff really engaged with this activity. Once they relaxed, and got going, they couldn\u2019t wait to tell us about issues related to getting a good night\u2019s sleep in the hospital and how we could use research to improve patients\u2019 sleep. One of the main issues raised was the amount of noise. Noises ranged from the window slats being opened every hour at night for safety checks, to staff talking at night unaware their voices echoed down the corridor.<\/p>\n<figure id=\"attachment_2143\" aria-describedby=\"caption-attachment-2143\" style=\"width: 640px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"576\" class=\"size-large wp-image-2143\" src=\"https:\/\/blogs.imperial.ac.uk\/ighi\/files\/2020\/02\/Lindsay-sleep-cafe-1024x576.jpg\" alt=\"Lindsay and a colleague at the sleep workshop\" \/><figcaption id=\"caption-attachment-2143\" class=\"wp-caption-text\">Lindsay (left) with Amanda hosting the Caf\u00e9.<\/figcaption><\/figure>\n<h2>A huge success<\/h2>\n<figure id=\"attachment_2145\" aria-describedby=\"caption-attachment-2145\" style=\"width: 169px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" width=\"169\" height=\"300\" class=\"wp-image-2145 size-medium\" src=\"https:\/\/blogs.imperial.ac.uk\/ighi\/files\/2020\/02\/Notes-from-sleep-workshop-169x300.jpg\" alt=\"Post it notes from the sleep workshop\" \/><figcaption id=\"caption-attachment-2145\" class=\"wp-caption-text\">Feedback from the event<\/figcaption><\/figure>\n<p>We expected there to be numerous barriers to conducting this event in the secure hospital because of the hospital security regulations. But thanks to considerable prep work and discussions with the security staff, many things we anticipated could raise problems weren\u2019t an issue. For example, pens were counted before and after the event, we used sticky Velcro rather than Blu Tac to reduce security risks and we all had to be escorted within the setting.<\/p>\n<p>The feedback from the attendees and our team shows the event was a huge success. Everyone said the event was helpful for learning something new about sleep, with people describing it as \u201cinformative\u201d, \u201cengaging\u201d and \u201cinteresting\u201d. Reflecting on the event, <a href=\"https:\/\/www.york.ac.uk\/healthsciences\/our-staff\/amanda-perry\/\">Dr Amanda Perry<\/a> from the University of York said, \u201cTalking to patients and staff over coffee was an opportunity to share ideas, worries and challenges in relation to the management of sleep in secure environments. Everyone I spoke to had a vested interest in making sleep better for all patients in this environment.\u201d<\/p>\n<p><a href=\"https:\/\/www.york.ac.uk\/healthsciences\/our-staff\/honoraryemeritus\/anne-aboaja\/\">Dr Anne Aboaja<\/a>, Forensic Psychiatrist at TEWV NHS Foundation Trust and Forensic Sleep Research Group lead said, \u201cThe People\u2019s Research Caf\u00e9 experience was successful, and it was great to learn about this methodology.\u201d<\/p>\n<h2>Next steps<\/h2>\n<p>We will now collate everything we learnt from the People\u2019s Research Caf\u00e9 into usable data to inform our research grant proposal. We\u2019re currently writing a commissioned piece of work on the topic of sleep in mental health settings for the journal <em>BJPsych Advances<\/em> and conducting a scoping review on sleep interventions for the same setting. Our protocol is under review at <em>JBI Evidence Synthesis<\/em>.<\/p>\n<p>We also hope to hold a similar in the future but covering multiple smaller research projects in one go.
