{"id":688,"date":"2016-11-11T14:44:37","date_gmt":"2016-11-11T14:44:37","guid":{"rendered":"https:\/\/imperialighi.wordpress.com\/?p=688"},"modified":"2019-03-29T10:48:28","modified_gmt":"2019-03-29T10:48:28","slug":"biotope-biomarkers-to-diagnose-pneumonia","status":"publish","type":"post","link":"https:\/\/blogs.imperial.ac.uk\/ighi\/2016\/11\/11\/biotope-biomarkers-to-diagnose-pneumonia\/","title":{"rendered":"BIOTOPE (BIOmarkers TO diagnose PnEumonia)"},"content":{"rendered":"<p><strong><em>By <a href=\"http:\/\/www.imperial.ac.uk\/people\/j.odonoghue\">Dr John O\u2019Donoghue<\/a>, Senior Lecturer in eHealth &amp; Deputy Director of Imperial\u2019s <a href=\"https:\/\/www1.imperial.ac.uk\/ehealth\/\">Global eHealth Unit<\/a><\/em><\/strong><\/p>\n<figure id=\"attachment_691\" aria-describedby=\"caption-attachment-691\" style=\"width: 256px\" class=\"wp-caption alignleft\"><a href=\"https:\/\/imperialighi.files.wordpress.com\/2016\/11\/jod_3-jpg.jpeg\"><img decoding=\"async\" class=\" wp-image-691\" src=\"https:\/\/imperialighi.files.wordpress.com\/2016\/11\/jod_3-jpg.jpeg\" alt=\"Mr Masters Chisale (Mzuzu Central Hospital) and Dr Chris Watson (Queens Universty Belfast) preparing the samples in Mzuzu Central Hospital. \" \/><\/a><figcaption id=\"caption-attachment-691\" class=\"wp-caption-text\">Mr Masters Chisale (Mzuzu Central Hospital) and Dr Chris Watson (Queens Universty Belfast) preparing the samples in Mzuzu Central Hospital.<\/figcaption><\/figure>\n<p>The last week has been very busy in Mzuzu, northern Malawi.\u00a0 Scientists there have been packing blood and urine samples collected from 506 children with pneumonia in preparation for shipment to Dublin, Ireland.\u00a0 These samples will travel 12,000km at -80<sup>o<\/sup>C with constant monitoring of their temperature and dry ice being packed around them at stops along the journey to ensure they remain frozen in the warm heat of Africa as they travel across the African and European continents.<\/p>\n<p>Over the past twelve months the researchers from the <a href=\"https:\/\/www.ucd.ie\/medicine\/ourresearch\/researchgroups\/ghealthresearchgroup\/\">gHealth Research group based in University College Dublin<\/a>, <a href=\"http:\/\/www.qub.ac.uk\/\">Queens University Belfast<\/a> &amp; Imperial College London have been working with colleagues in Malawi to collect these samples.\u00a0 The BIOTOPE (BIOmarkers TO diagnose PnEumonia) project is an innovative project which received funding from the Gates Foundation Grand Challenges Exploration Fund to investigate new ways to diagnose bacterial and severe pneumonia in the community. 506 children have participated in the research and as part of this, blood and urine samples have been collected and are now being shipped to Ireland for detailed analysis along with novel data from sensors and mobile phone applications monitoring the children\u2019s health.\u00a0 With this approach, new ways to help stop the million children dying each year from pneumonia are being developed along with approaches that will help reduce antibiotic resistance both in Africa and internationally.<\/p>\n<p>Pneumonia continues to be the number one infectious killer of children under the age of 5 years worldwide \u2013 more than HIV, TB, Zika, Ebola and malaria combined.<\/p>\n<p>People of any age in any country are at risk of contracting pneumonia but the vast majority (&gt;90%) of childhood deaths from pneumonia occur in poor countries.<\/p>\n<p>Antibiotic resistance is present in every country in the world and as recently as the 21<sup>st<\/sup> September 2016 the UN held a general assembly on antibiotic resistance.\u00a0 This was only the fourth such General Assembly high-level event in the history of the UN to\u00a0focus on a health issue highlighting the serious nature of this problem.<strong>\u00a0<\/strong><!--more--><\/p>\n<p><strong>Background<\/strong><\/p>\n<p>Each year it is estimated that 5.9 million deaths occur in children aged less than 5 years of age.\u00a0 Pneumonia is one of the major causes accounting for approximately 16% of these deaths \u2013 that is one child dying every twenty seconds from pneumonia.\u00a0 Pneumonia occurs when the lungs become infected &#8211; usually with bacteria or viruses.\u00a0 It made headlines recently when Hilary Clinton developed it during her campaign for US president.\u00a0 Although often easily treated in developed countries difficulties in accessing healthcare, diagnostics and treatment mean that many children die from pneumonia each year in poorer countries.<\/p>\n<p>Current guidelines in Africa suggest that all children with pneumonia should be treated with antibiotics. Antibiotics improve outcomes in children with pneumonia caused by bacteria.\u00a0 However only approximately 20% of children with pneumonia have a bacterial infection. Since it is still impossible to clinically differentiate viral from bacterial pneumonia, prompt treatment of pneumonia with antibiotics remains a priority due to the high mortality rates. Concern now arises regarding the widespread use of antibiotics in those with viral infections leading to increased antibiotic resistance. Better use of antibiotics targeted at those with bacterial infections will help reduce antibiotic resistance.<\/p>\n<figure id=\"attachment_690\" aria-describedby=\"caption-attachment-690\" style=\"width: 233px\" class=\"wp-caption alignright\"><a href=\"https:\/\/imperialighi.files.wordpress.com\/2016\/11\/jod_2.jpg\"><img decoding=\"async\" class=\"wp-image-690\" src=\"https:\/\/imperialighi.files.wordpress.com\/2016\/11\/jod_2.jpg\" alt=\"jod_2\" \/><\/a><figcaption id=\"caption-attachment-690\" class=\"wp-caption-text\">Dr Joe Gallagher (UCD\/ICL) with Mr Masters Chisale (Mzuzu Central Hospital) and Dr Chris Watson (Queens Universty Belfast).<\/figcaption><\/figure>\n<p>A biomarker is a naturally occurring molecule, gene, or characteristic by which a particular disease can be identified. An ideal biomarker for pneumonia would allow an early diagnosis of the condition and its severity and identify whether it is caused by bacteria or viruses. BIOTOPE is looking at the response of the body\u2019s immune system to infection to help identify bacterial and severe pneumonia.\u00a0 Using the samples shipped back to Ireland Dr Chris Watson and his team will screen the samples using an \u2013omics approach involving proteomics, metabolomics and microRNAs which will identify which children would benefit from antibiotics. Dr Joe Gallagher, Honorary Lecturer in Primary Care at ICL and Dr John O Donoghue Senior Lecturer in eHealth and Deputy Director of the Global eHealth Unit in ICL will use the clinical data collected on the symptom and examination findings as well as the sensor data to find ways to identify the most severely ill children.\u00a0 Combining these datasets will allow the creation of new tools that can be deployed on mobile phones in Africa to help diagnose childhood pneumonia and reduce antibiotic resistance.
