high-throughput instrument with a good computer-based laboratory management system, it can take months for the results to get back to him.\u00a0 Sometimes, they go missing. <\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><!--more--><\/p>\n<p><span data-contrast=\"auto\">Meet Prossy Nantale, she lives in a fishing village off the coast of Lake Victoria.\u00a0 It is a two-hour canoe trip to Jinja.\u00a0 The doctor gives her a 6-month supply of generic drugs when she goes in for her HIV visits.\u00a0 This is when he gives her the results from her last viral load test and draws her blood for another test.\u00a0 If the results are mislaid, it will be a year, maybe more, before she knows whether she has developed drug-resistance and if she needs to change her treatment regimen.\u00a0 She is a widow who cares for an elderly mother and three children.\u00a0 She often skips her medicine and does not keep to her doctor\u2019s visits very well, because the district hospital is so far away, and she needs the little money she has for other things.\u00a0 She doesn\u2019t go unless she is very sick or if she is in Jinja for something else and can afford the visit.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Meet Jalil Ahmed, he is a clinician in Ratodero, a small town in southeastern Pakistan where there is a growing HIV-1 outbreak.\u00a0 There is no treatment or testing facility for children here or in the nearest large town, Larkana, 20 miles away.\u00a0 Children must go to Karachi, 300 miles away from treatment and monitoring.\u00a0 They are not diagnosed early, and drug-resistance is not detected until it is too late.\u00a0 Their families simply cannot afford the trips to Karachi or the medications.\u00a0 <\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">A surge in drug resistance<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">There are currently 38 million people infected with HIV worldwide with up to a million deaths each year.\u00a0 There is a very concerning increase in the development of HIV-1 drug resistance in low and middle-income countries.\u00a0 Over 80% of HIV positive individuals know their status and are on treatment in East and Southern Africa for example.\u00a0 Less than 45% of them have access to treatment-monitoring, however, which is causing this surge in drug resistance.\u00a0 Up to 69% of infants born to HIV positive mothers harbour strains that are resistant to the only drugs readily available to them in the worst-affected areas.\u00a0 In Pakistan, there are approximately 190,000 HIV positive people.\u00a0 Only 21% know their status, of whom only 12% are on treatment.\u00a0 There has been a 385% increase in HIV-related deaths in Pakistan since 2010 as a result.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">My name is <\/span><a href=\"https:\/\/www.imperial.ac.uk\/people\/c.kibirige\"><span data-contrast=\"none\">Dr. Catherine Kibirige<\/span><\/a><span data-contrast=\"auto\">.\u00a0 I am a scientist with over 20 years\u2019 experience in HIV research.\u00a0 These are the stories that drive my work and keep me up late in the laboratory.\u00a0 Prior to beginning graduate studies, I worked under the Ministry of Health in Uganda on a study of HIV\/AIDS and related STDs.\u00a0 My PhD thesis work was conducted within the Department of Molecular Microbiology and Immunology at the Johns Hopkins Bloomberg School of Public Health. Baltimore, MD, USA.\u00a0 I drew my patient samples from and worked alongside the Multi-Center AIDS Cohort Study (MACS) Washington\/Baltimore arm.\u00a0 I then went on to conduct a post-doctoral fellowship with the US Military\u2019s HIV Research Program working within the Technology Assessment Laboratory at the Walter Reed Army Institute of Research in Maryland, USA.\u00a0 I worked on a series of projects developing molecular diagnostic assays capable of early HIV detection.\u00a0 I attempted to cover all known subtypes and variants of HIV-1.\u00a0 Towards the end of this position, I wrote a series of proposals and successfully obtained a technology acceleration seed grant through the Johns Hopkins technology transfer department which I used to further optimize the assays and broaden their utility during a second postdoctoral position back at Johns Hopkins.\u00a0 I presented my work at the Annual Vaccine Research and Development conference in Baltimore in November 2015, which led to my eventual position with what was formerly the International AIDS Vaccine Initiative (<\/span><a href=\"https:\/\/www.iavi.org\/\"><span data-contrast=\"none\">IAVI<\/span><\/a><span data-contrast=\"auto\">), <\/span><a href=\"https:\/\/www.imperial.ac.uk\/infectious-disease\/research\/immunology-infection\/human-immunology\/\"><span data-contrast=\"none\">Human Immunology Laboratory<\/span><\/a><span data-contrast=\"auto\"> here at Imperial College London.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">Public health concern<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"none\">HIV-1 suppression, following initiation of <\/span><span data-contrast=\"auto\">combination antiretroviral therapy (cART)<\/span><span data-contrast=\"none\"> to levels that are undetectable by commercial viral load assays (i.e., below 20 to 50 copies\/ml in plasma), is significantly associated with longevity and improved quality of life.\u00a0 <\/span><span data-contrast=\"auto\">Moreover, since successfully treated people do not transmit the virus, early and sustained viral suppression is an important public health tool in the prevention HIV spread.\u00a0 <\/span><a href=\"https:\/\/www.who.int\/hiv\/pub\/arv\/arv-2016\/en\/\"><span data-contrast=\"none\">Current WHO guidelines<\/span><\/a><span data-contrast=\"auto\"> strongly recommend initiation of cART as soon as possible after diagnosis (WHO 2016).\u00a0 Current HIV-1 viral load assays, which are used to monitor treatment, range from \u00a350 to \u00a3100 a test.\u00a0 The assays require a cold-chain, specialized equipment, and specially trained staff.\u00a0 The more widespread dissemination and use of low-cost generic first-line HIV-1 drugs means that treatment is becoming more widely available worldwide, but the lack of adequate monitoring is causing the emergence and circulation of treatment-resistant strains of the virus<\/span> <span data-contrast=\"auto\">as we have seen in the case studies above.\u00a0 This is a major public health concern.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Despite many years of calls in the literature, to validate and utilize low-cost laboratory developed assays (LDAs), such as mine, for treatment monitoring and despite the development of programs that facilitate the validation of such assays at the same stringency as commercially approved assays, wide-spread use of LDAs for treatment monitoring has not come to fruition because it is a long-winded, expensive, and painstaking process.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335551550&quot;:6,&quot;335551620&quot;:6,&quot;335559731&quot;:720,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h2><b><span data-contrast=\"auto\">It has become my obsession<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/h2>\n<p><span data-contrast=\"auto\">Over the past 10 years, I have become frustrated with this scenario and have determined to do it myself. It has become an obsession. <\/span><span data-contrast=\"auto\">I have <\/span><a href=\"https:\/\/soundcloud.com\/user-499485249\/simple-hiv-test?\"><span data-contrast=\"none\">developed, patented, and published a novel assay<\/span><\/a><span data-contrast=\"auto\"> for the quantification of HIV-1 DNA in cells with a sensitivity of a single infected cell or 3 copies of DNA at the 95% confidence level.\u00a0 Unlike competing laboratory-developed assays, all major HIV-1 subtypes, and a wide range of recombinants from a 127-member internationally recognized diversity panel were detected and accurately quantified.\u00a0 The assay has been evaluated according to international guidelines using an extensive diversity panel of HIV-1 strains provided by the external quality assurance program oversight laboratory (EQAPOL) and the AIDS Reference Reagent Program.
