To mark World Head and Neck Cancer Day, we hear from Grainne Brady, PhD Student at Imperial\u2019s Department of Surgery and Cancer, NIHR Clinical Doctoral Research Fellow and Speech Language Therapist at the Royal Marsden NHS Foundation Trust London. She shares her expertise on the link between head and neck cancer and changes to swallowing, communication and the potential impact on quality of life.\u00a0<\/strong><\/p>\n<hr \/>\n<p><span data-contrast=\"auto\">Treatments for HNC include s<\/span><span data-contrast=\"auto\">urgery, radiation and chemotherapy<\/span><span data-contrast=\"auto\">(1)<\/span><span data-contrast=\"auto\">. <\/span><span data-contrast=\"auto\">Traditional risk factors for HNC include a history of tobacco and alcohol use <\/span><span data-contrast=\"auto\">(2)<\/span><span data-contrast=\"auto\">. More recently, human papilloma virus (HPV) has been recognised as a risk factor for the development of oropharyngeal cancer which is a subtype of cancer affecting the back of the throat, tonsils, back of the tongue and soft palate <\/span><span data-contrast=\"auto\">(2)<\/span><span data-contrast=\"auto\">. T<\/span><span data-contrast=\"auto\">he <\/span><span data-contrast=\"auto\">discovery<\/span><span data-contrast=\"auto\"> that a virus can cause this condition<\/span> <span data-contrast=\"auto\">has led to the development of a UK-wide HPV vaccination programme available for both boys and girls aged 12-13 years <\/span><span data-contrast=\"auto\">(3)<\/span><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559740&quot;:480}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">There have been huge advances in the treatment of HNC over the past two<\/span><span data-contrast=\"auto\"> to <\/span><span data-contrast=\"auto\">&#8211; <\/span><span data-contrast=\"auto\">three decades<\/span><span data-contrast=\"auto\">. <\/span><span data-contrast=\"auto\">These include advanced radiation techniques like intensity-modulated radiation therapy and minimally invasive surgeries such as transoral robotic surgery, which are now<\/span><span data-contrast=\"auto\"> standard of care techniques<\/span> <span data-contrast=\"auto\">(4)<\/span><span data-contrast=\"auto\">.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559740&quot;:480}\">\u00a0<\/span><\/p>\n<p><!--more--><\/p>\n<p><span data-contrast=\"auto\">HNC and the anti-cancer treatments administered can result in significant changes to an individual\u2019s ability to eat, drink and swallow safely and comfortably and to be able to produce clear voice and speech. Difficulties with swallowing (dysphagia)<\/span><span data-contrast=\"auto\"> has been highlighted as a key priority concern at 12 months following treatment for HNC <\/span><span data-contrast=\"auto\">(5)<\/span><span data-contrast=\"auto\">. Dysphagia<\/span><span data-contrast=\"auto\"> can lead to significant health <\/span><span data-contrast=\"auto\">complications including malnutrition, dehydration, pneumonia, feeding tube <\/span><span data-contrast=\"auto\">dependency<\/span><span data-contrast=\"auto\">,<\/span><span data-contrast=\"auto\"> and significant healthcare costs. Both changes to swallowing and\/or a person\u2019s ability to communicate can have a significant impact on their overall quality of life (QoL). <\/span><span data-contrast=\"auto\">Given the potential impact of HNC on swallowing and communication and <\/span><span data-contrast=\"auto\">subsequent<\/span><span data-contrast=\"auto\"> QoL, there has been <\/span><span data-contrast=\"auto\">a paradigm shift<\/span><span data-contrast=\"auto\"> in HNC research to not only focus on survival outcomes, but to place equal importance on function and QoL outcomes for our patients<\/span><span data-contrast=\"auto\">.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559740&quot;:480}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">As a speech <\/span><span data-contrast=\"auto\">and<\/span><span data-contrast=\"auto\"> language therap<\/span><span data-contrast=\"auto\">ist<\/span><span data-contrast=\"auto\"> working <\/span><span data-contrast=\"auto\">in the area of<\/span><span data-contrast=\"auto\"> HNC, my role is to assess, diagnose and treat any communication or swallowing difficulties which arise <\/span><span data-contrast=\"auto\">as<\/span><span data-contrast=\"auto\"> a result of<\/span><span data-contrast=\"auto\"> the HNC and\/or the anti-cancer treatment(s) adminis<\/span><span data-contrast=\"auto\">tered. <\/span><span data-contrast=\"auto\">I have always had a keen interest in clinically based research aiming to optimise the function, QoL and patient experience outcomes for the patients I treat clinically<\/span><span data-contrast=\"auto\">.<\/span><span data-contrast=\"auto\">\u00a0<\/span> <span data-contrast=\"auto\">I<\/span><span data-contrast=\"auto\"> was delighted to be accepted as a PhD candidate within the Department of Surgery <\/span><span data-contrast=\"auto\">and<\/span><span data-contrast=\"auto\"> Cancer at Imperial College London. <\/span><span data-contrast=\"auto\">Working alongside my supervisors at Imperial<\/span><span data-contrast=\"auto\"> \u2013 <\/span><span data-contrast=\"auto\">Professor Mary Wells, Professor Justin <\/span><span data-contrast=\"auto\">Roe<\/span><span data-contrast=\"auto\"> and Professor <\/span><span data-contrast=\"auto\">Pernilla<\/span> <span data-contrast=\"auto\">Lagergren<\/span><span data-contrast=\"auto\"> \u2013<\/span><span data-contrast=\"auto\"> in collaboration with Professor <\/span><span data-contrast=\"auto\">Vinidh<\/span> <span data-contrast=\"auto\">Paleri<\/span><span data-contrast=\"auto\"> at the Royal Marsden NHS Foundation Trust, I <\/span><span data-contrast=\"auto\">am currently completing my PhD research which <\/span><span data-contrast=\"auto\">is<\/span><span data-contrast=\"auto\"> investigat<\/span><span data-contrast=\"auto\">ing<\/span><span data-contrast=\"auto\"> function and QoL outcomes <\/span><span data-contrast=\"auto\">and patient and carer experience in\u00a0 recurrent HNC.
