As interest grows globally in the sector, our understanding of the complex and ancient evolutionary physiological strategies involved, and their impact on infant and maternal health, deepens. The maternal health impacts of lactation and how to best support families are the key focuses of my research, and like much of the work across women\u2019s health, has been largely ignored by mainstream science. <\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:278}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">In partnership with <a href=\"https:\/\/www.google.com\/url?sa=t&amp;source=web&amp;rct=j&amp;opi=89978449&amp;url=https:\/\/www.georgeinstitute.org.uk\/&amp;ved=2ahUKEwjrlIPDhNGHAxVvS0EAHR5APa4QFnoECBUQAQ&amp;usg=AOvVaw1tn9k2-AhRRJiMdL5nBiJr\">The Ge<\/a><\/span><a href=\"https:\/\/www.google.com\/url?sa=t&amp;source=web&amp;rct=j&amp;opi=89978449&amp;url=https:\/\/www.georgeinstitute.org.uk\/&amp;ved=2ahUKEwjrlIPDhNGHAxVvS0EAHR5APa4QFnoECBUQAQ&amp;usg=AOvVaw1tn9k2-AhRRJiMdL5nBiJr\"><span data-contrast=\"auto\">o<\/span><span data-contrast=\"auto\">r<\/span><\/a><span data-contrast=\"auto\"><a href=\"https:\/\/www.google.com\/url?sa=t&amp;source=web&amp;rct=j&amp;opi=89978449&amp;url=https:\/\/www.georgeinstitute.org.uk\/&amp;ved=2ahUKEwjrlIPDhNGHAxVvS0EAHR5APa4QFnoECBUQAQ&amp;usg=AOvVaw1tn9k2-AhRRJiMdL5nBiJr\">ge <\/a><a href=\"https:\/\/www.google.com\/url?sa=t&amp;source=web&amp;rct=j&amp;opi=89978449&amp;url=https:\/\/www.georgeinstitute.org.uk\/&amp;ved=2ahUKEwjrlIPDhNGHAxVvS0EAHR5APa4QFnoECBUQAQ&amp;usg=AOvVaw1tn9k2-AhRRJiMdL5nBiJr\">Institute<\/a>, the new <\/span><a href=\"https:\/\/www.imperial.ac.uk\/womens-health-network\/\">Women\u2019s Health Network<\/a> at Imperial College London is seeking to correct these crucial gaps. Excitingly, our study into the mechanisms through which breastfeeding can reduce the risk of certain types of cancer has received further funding from <a href=\"https:\/\/breastcancernow.org\/about-us\/media\/press-releases\/new-research-to-investigate-why-breastfeeding-reduces-the-risk-of-developing-breast-cancer\/\">Breast Cancer Now<\/a><span data-contrast=\"auto\">. Yet, there is still so much to discover. Just last month an entirely new hormonal response that explains how breastfeeding protects against osteoporosis was described by scientists at UC Davis. They found bone metabolism increases during lactation to keep up with the large amount of calcium required to produce milk, at the same time as levels of the protective hormone oestrogen fall. Scientists were therefore previously stumped as to how breastfeeding prevents rather than causes maternal osteoporosis. The work, published in <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41586-024-07634-3\"><b><span data-contrast=\"none\">Nature<\/span><\/b><\/a><span data-contrast=\"auto\">, found that during breastfeeding, certain neurons in the brain called KISS1 neurons release a protein called CCN3. This protein helps increase bone repair, boosts the activity of skeletal stem cell activity and even speeds up how quickly fractures repair.<\/span><\/p>\n<p><!--more--><\/p>\n<h2>The reality &#8216;on the ground&#8217;<\/h2>\n<p><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW142715536 BCX4\">W<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">hile the science has been awe-inspiring, the <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">reality on the ground <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">remains<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> dire.<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> Families <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">clearly <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">understand the <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">benefits<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> of breastfeeding, but when it comes to trying to feed their babies, just <\/span><span class=\"NormalTextRun CommentStart CommentHighlightPipeRestV2 CommentHighlightRest SCXW142715536 BCX4\">about <\/span><\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentHighlightRest SCXW142715536 BCX4\">every barrier<\/span> <\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun CommentHighlightRest SCXW142715536 BCX4\">possible has been <\/span><span class=\"NormalTextRun CommentHighlightRest SCXW142715536 BCX4\">created in the UK <\/span><span class=\"NormalTextRun CommentHighlightPipeRestV2 SCXW142715536 BCX4\">over the last 70 years. These<\/span>\u00a0<span class=\"NormalTextRun SCXW142715536 BCX4\">range from a loss of shared cultural <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">experience and knowledge<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">, early <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">postnatal <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">discharge<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">, a lack of ubiquitous early specialist lactation support, unsupportive employment practices<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">,<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> insufficient screening tools to detect <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">under<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">lying <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">pat<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">hology<\/span> <span class=\"NormalTextRun SCXW142715536 BCX4\">that can affect milk supply, an<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">d many <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">more. These barriers and others combine to mean that <\/span><\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW142715536 BCX4\">nine out of ten women stop feeding before they planned<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> by 6-8 weeks<\/span><\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW142715536 BCX4\">. <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">T<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">he <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">skilled knowledge and <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">practical support <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">needed <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">to help families <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">establish<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> and maintain breastfeeding <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">in the UK <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">remain<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">s<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> splintered, under-resourced and<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">,<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> anecdotally at least, <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">even <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">less <\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW142715536 BCX4\">a<\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW142715536 BCX4\">ccessible <\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW142715536 BCX4\">than<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> before the pandemic. Breastfeeding rates in many parts of the country, and particularly those in deprived areas, appear to be <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">falling again. <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">The exception is <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">Scotland, where the devolved government has chosen to invest into peer support, milk banking services, and critically, data collection<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> \u2013 the success<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">ful outcomes of these are<\/span><span class=\"NormalTextRun SCXW142715536 BCX4\"> seen in <\/span><span class=\"NormalTextRun SCXW142715536 BCX4\">raised initiation rates and increased rates of any breast<\/span><\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW142715536 BCX4\">feeding at least up to 13 months<\/span><\/span><span class=\"TextRun SCXW142715536 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW142715536 BCX4\">.<\/span><\/span><span class=\"EOP SCXW142715536 BCX4\" data-ccp-props=\"{&quot;134233117&quot;:false,&quot;134233118&quot;:false,&quot;201341983&quot;:0,&quot;335551550&quot;:1,&quot;335551620&quot;:1,&quot;335559685&quot;:0,&quot;335559737&quot;:0,&quot;335559738&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:278}\">\u00a0<\/span><\/p>\n<p><span class=\"TextRun SCXW67023755 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW67023755 BCX4\">But is hope <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">for families in other devolved nations <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">on the horizon<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">? <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">The last government invested \u00a350 million into the new <\/span><\/span><a class=\"Hyperlink SCXW67023755 BCX4\" href=\"https:\/\/familyhubsnetwork.com\/\" target=\"_blank\" rel=\"noreferrer noopener\"><span class=\"FieldRange SCXW67023755 BCX4\"><span class=\"TextRun Underlined SCXW67023755 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW67023755 BCX4\" data-ccp-charstyle=\"Hyperlink\">Family \u00a0Hubs network<\/span><\/span><\/span><\/a><span class=\"TextRun SCXW67023755 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW67023755 BCX4\">, aiding pilot projects to support breastfeeding<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">.<\/span> <span class=\"NormalTextRun SCXW67023755 BCX4\">How<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">ever,<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\"> without the use of an evidence-based strategy to support best-practice implementation<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">,<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\"> it is not <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">yet <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">clear how well that money has been spent.<\/span> <span class=\"NormalTextRun SCXW67023755 BCX4\">So, f<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">or the first time in 15 years, <\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW67023755 BCX4\">t<\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW67023755 BCX4\">he<\/span> <\/span><a class=\"Hyperlink SCXW67023755 BCX4\" href=\"https:\/\/www.gov.uk\/guidance\/infant-feeding-survey-2023\" target=\"_blank\" rel=\"noreferrer noopener\"><span class=\"TextRun Underlined SCXW67023755 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"none\"><span class=\"NormalTextRun SCXW67023755 BCX4\" data-ccp-charstyle=\"Hyperlink\">new Infant Feeding Survey in England<\/span><\/span><\/a><span class=\"TextRun SCXW67023755 BCX4\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW67023755 BCX4\"> will <\/span><span class=\"NormalTextRun SCXW67023755 BCX4\">report next <\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW67023755 BCX4\">year<\/span><span class=\"NormalTextRun ContextualSpellingAndGrammarErrorV2Themed SCXW67023755 BCX4\">,<\/span><span class=\"NormalTextRun SCXW67023755 BCX4\"> and this data will be invaluable to the planning and implementation of services<\/span><\/span><\/p>\n<h2>Equitable milk bank services<\/h2>\n<p>My own programme of research has focused specifically on human milk banks, a stretched orphan service analogous to blood transfusion services, which provides screened donor human milk to feed vulnerable infants when their own mothers cannot provide any or enough of their own milk. In collaboration with Professor Amy Brown at Swansea University, work published this year has shown that where donor milk was available as a bridge to breastfeeding, or where breastfeeding was impossible for health reasons such as maternal cancer, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9154035\/\">parental wellbeing<\/a> and <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/mcn.13686\">depression and anxiety scores normalised<\/a>. Moreover, Professor Brown has shown that <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/mcn.13699\">women who donated their milk<\/a> found the experience improved their own wellbeing, particularly those who had experienced birth trauma, a neonatal intensive care stay, or the tragedy of the death of a baby. They described being able to donate their milk as giving them a sense of purpose, maintaining a connection with their baby, and taking control back over their body after very difficult birth experiences. Those women who were unable to donate, often because milk banks were too small to accept milk, described unexpectedly severe and long-lasting negative emotions. This finding surely adds additional weight to the increased resourcing of milk bank services in the UK, to provide truly equitable services that can support families through both donating and receiving donor milk.<\/p>\n<p>Excitingly, one county council, Norfolk, has recently commissioned the Human Milk Foundation to increase access to milk bank services across the county through their <a href=\"https:\/\/www.norfolk.gov.uk\/startforlife\">Start for Life<\/a> programme. Over the next year, we will be evaluating how this commissioning translates to more donors recruited, more families supported, and impact on breastfeeding perceptions and rates. Initially, small quantities of donor milk will be made available on the postnatal wards in Norfolk to families where the mother has diabetes during pregnancy. Being breastfed <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/apa.13133\">reduces the risk of children developing diabetes<\/a>, and <a href=\"https:\/\/journals.sagepub.com\/doi\/10.1177\/0890334420950259?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed\">speeds up how quickly maternal metabolism returns to normal after birth<\/a>, protecting women with gestational diabetes from developing type II diabetes later in life. However, diabetes in pregnancy can <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/dme.12846\">make establishing breastfeeding more challenging<\/a>, and babies are often given supplemental infant formula. Women are less likely to establish breastfeeding as a result; however previous research has shown women are more likely to see donor milk as a supportive bridge to give them the necessary time until their milk comes in. The results of this innovative pilot project will give valuable information for the planning of a future multicentre clinical trial.<\/p>\n<p>Our work and that of others round the world is showing how significant <a href=\"https:\/\/internationalbreastfeedingjournal.biomedcentral.com\/articles\/10.1186\/s13006-021-00446-5\">access to donor milk can be for families facing wide varieties of challenges<\/a> with feeding their babies. Given that milk bank services are over 85 years old in the UK, it is perhaps surprising that the <a href=\"https:\/\/www.bapm.org\/resources\/the-use-of-donor-human-milk-in-neonates\">first UK Framework for Practice guidelines<\/a> for use were only published in 2023, limited to the use of donor human milk in hospitals. These guidelines made recommendations that babies born very prematurely, less than 32 weeks or weighing less than 1500 grams should have access. But these guidelines fell short of the recent <a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240010765\">Standards for the Care of the Small and Sick Newborns<\/a> published by the World Health Organization (WHO) which stated donor milk should be available for all premature infants in the absence or shortfall of maternal milk. Having sat on the British Association of Perinatal Medicine\u2019s working group for the year leading up to the publication, the unfortunate truth is that UK milk banks are currently stretched in terms of staffing and resourcing, and vulnerable to external pressures such as the fuel crisis, and cost of living crisis, as well as the lasting effects of the COVID-19 <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/mcn.13131\">pandemic<\/a>, during which it took a <a href=\"https:\/\/www.infantjournal.co.uk\/pdf\/inf_093_7153.pdf\">monumental collective effort to maintain services<\/a>. <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0377221724003795\">Our modelling work<\/a> with the University of Exeter suggests that broadening access criteria to babies born less than 34 weeks gestation (two months early) would need milk banks to triple in size. Given how often <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/mcn.13526\">existing services face donor milk shortages<\/a> even now, major investment is necessary in staffing, training and service resilience before the UK can hope to meet the WHO recommendations.
